Non-invasive method and apparatus for monitoring the condition of a heart failure patient and for optimizing the pacing parameters of a cardiac device implanted in a patient. A plethysmogram signal, e.g., a finger photoplethysmogram, is obtained from a patient and provided to a programmer device. The plethysmogram signal is analyzed by the programmer device to obtain a cardiac performance parameter, e.g., a pulse amplitude response, a degree of pulsus alternans, or irregularity in the pressure pulses detected in an atrial fibrillation patient. The effect on the cardiac performance parameter derived from the plethysmogram is determined for various pacing parameter values in a manner so as to reject noncardiogenic effects and artifacts. Pacing parameters resulting in the best cardiac performance parameter may be selected as the optimum pacing parameters. The programmer device may monitor a Valsalva maneuver performed by a patient. Optimum pacing parameters may be derived by analysis of the plethysmogram signals obtained during performance of the Valsalva maneuver using different pacing parameters.

Méthode non envahissante et appareil pour surveiller l'état d'un patient d'arrêt du coeur et pour optimiser les paramètres arpentants d'un dispositif cardiaque implanté dans un patient. Un signal de plethysmogram, par exemple, un photoplethysmogram de doigt, est obtenu à partir d'un patient et si à un dispositif de programmeur. Le signal de plethysmogram est analysé par le dispositif de programmeur pour obtenir un paramètre d'exécution cardiaque, par exemple, une réponse d'amplitude d'impulsion, un degré d'alternans de pulsus, ou irrégularité dans les impulsions de pression détectées dans un patient de fibrillation atriale. L'effet sur le paramètre d'exécution cardiaque dérivé à partir du plethysmogram est déterminé pour différentes valeurs de paramètre arpentantes en quelque sorte afin de rejeter des effets et des objets façonnés non cardiogéniques. Des paramètres arpentants ayant pour résultat le meilleur paramètre d'exécution cardiaque peuvent être choisis comme paramètres arpentants optima. Le dispositif de programmeur peut surveiller une manoeuvre de Valsalva exécutée par un patient. Des paramètres arpentants d'optimum peuvent être dérivés par l'analyse des signaux de plethysmogram obtenus pendant l'exécution de la manoeuvre de Valsalva en utilisant différents paramètres arpentants.
