An information storage device having a first part such as an insert on
which information is provided and a second part comprising a holder for
the first part, whereby the first part may be stored in the second part.
The edges of the first and second parts may be provided with lips for
mating together to provide interlocking parts whereby the first part may
be pulled outwardly to expose the information thereon but is prevented
from being completely separated from the holder.
